Dear Sir,
My wife Smt Vineeta Prakash and I (Sanjay Prakash s/o Shri P P Shrivastav, IAS (Retd), residing at Maatangi Bhawan, Chhatarpur Mandir Complex, Chhatarpur)had applied for issue of Voter’s Identity Card at the nearest Election Office near Dena Bank in Chhatarpur. The officer checked all papers along with Application Form and finding them complete gave two receipts on 28.6.08. Neither has the Voter’s ID Card been received nor has anything been heard from that office since then.
ID Card for my sister Ms Sadhna Shrivastav, whose application form was submitted along with those of ours, was received a couple of months back, but our IDs are still pending.
Please get the ID cards for both of us issued well before the election date.

I have applied for change of address in January 2009 for me and my family whose names figure at Sl. Nos. 434, 435, 436 of the voter list of assembly constituency 44, R.K. Puram. Address has been changed in the voter list but the voter i-cards has not yet been received by us till date. Also my son Himanshu Sharma who has completed 18 years of age has applied for new voter i-card. His name has been included in the voter list at Sl. No. 892 and alloted EPIC No.NLN 1479864 but the voter i-card has not yet been received. Kindly let me know the status.
